Roguelike dungeon crawler where you play Isaac, a kid fleeing his mother's deadly religious delusion. Each run sends you through procedurally generated basements packed with grotesque enemies and multi-phase bosses. Combat is twin-stick shooter style—tears are your weapon. Hundreds of items spawn randomly, stacking effects that warp both your stats and Isaac's physical form (extra heads, fly swarms, laser eyes). Runs rarely play the same twice because item synergies create wildly different builds. Secrets hide behind cracked walls and beneath rocks. Difficulty spikes hard as you descend. The art style mixes childlike drawings with body horror. Deaths are permanent but unlock new characters and items for future attempts. Expect 15-minute runs that either snowball into god-mode or end in the first few rooms.
